Does your child have any known health needs? e.g. diabetes, asthma, epilepsy, allergies. Please be as specific as possible if there are actions the Club should take (e.g. provision of medication) or if club members need any medical training other than First Aid to care for your child.
The full season fee for the 2023/24 season for U7 and above will remain at £200 and will be £60 for the U6 development squad.
For the 2023/24 season, your child will only be registered to play with the league once the subs have been paid.
If you are paying in instalments, you must contact the Club Treasurer (richard.turrington@gmail.com) to agree the timing of payments. Your child will only be registered with the league if the timing of payments has been agreed or the subs are paid in full.

Photos and ID - New players will also need to supply the team manager with a current photo (head and shoulders shot and proof of ID (passport or birth certificate)). Existing players may also need to provide an updated photo – please check the newsletter or confirm with your team manager for details.
Medical Consent
If it becomes necessary for my child to receive medical treatment and I cannot be contacted to authorise this, I hereby give my general consent to any necessary medical treatment and authorise the team manager/coach appointed by Reigate Football Club to sign any document required by the hospital authorities. Consent to Play
I understand that whilst every care will be taken by the officials of the club to ensure that players are properly looked after, it must be remembered that football is a physical game and from time to time injuries do occur. It is of course rare for these injuries to be more than superficial. However, it must be accepted that Reigate Football Club (including its volunteers) cannot be held responsible for any injuries that may be incurred. I give consent for my child to participate in Club activities/events and agree to the conditions outlined above. I accept that it is my responsibility to inform the Club directly of any changes to the details / circumstances recorded on this form. I also confirm that we have read and agree to the Club Code of Conduct. By selecting "I consent" below, I agree with the above.
We wish to promote positive images of young people playing football for Reigate Football Club. I give the Club permission to use images in newsletters, newspapers (U12 and above only), the Club’s website etc. promoting and celebrating the activities of the Club. Please give 7 days’ notice if you wish any image to be removed from the website.
By consenting under this section, you are confirming that you have read and understood the activities being offered to your child and agree with the measures the club has put in place to manage any risks, including its Covid-19 measures in line with current Government guidance.

If your child has any specific medical conditions, please ensure that you discuss these with the club welfare officer and or coach and agree the best way to support your child’s needs e.g. you staying to watch nearby and taking responsibility to administer the medication. If you wish to withdraw consent to your child participating in any or all activities, please notify the team coach in advance, or as soon as possible.
